Despite putting forth an effort to clear his brain right now, there's a lot going on in his head. Most of the thoughts are disjointed background voices that collide into each other like a dense asteroid field and sometimes get stuck and caught on a phrase like a broken record and then abandoned or lost. On top of that are lots of whispers and random phrases that sound like they're talking to Lance rather than being a thought he's consciously having.
Basically, his head sounds chaotic (crazy, probably). But through it all there's one clear thought that sticks out now that he's quieted as much a he can: 'last time this happened, he totally freaked. no don't- shut up.']
